  2. Save your receipts. A good receipt has the name of the product, date and cost. If it lacks any of these, please request a more detailed copy. If the product is not named on the receipt, a copy from a catalog page, web page or advertisement that lists the price might do (I just did this recently but not sure if I did it correctly).

  3. Fill out the XXX form. (Ideally we'll have a small image of the form here). This needs three bits of information entered: Type of thing purchased, company you purchased it from, total amount. Glue on the receipt in the box at the bottom of the from.

  4. Put it on XXX's desk where there is a box for reimbursement.
